• Facial Rejuvenation & Body Contouring

• Wellness Services including IV Therapy and BallancerPro

All services require prior consultation and may involve physician supervision. We do not offer virtual consultations at this time.

3. Eligibility & Client Requirements

• Clients must be at least 16 years old to receive services.
• All clients are required to complete medical consent forms before any treatment.
• We reserve the right to refuse service to anyone not meeting our eligibility criteria or behaving inappropriately.

4. Appointments, Deposits & Payment

• A deposit is required to confirm all appointments.
• A minimum 24-hour notice is required for cancellations or rescheduling.
• No-shows or late cancellations will incur a $50 fee.
• We accept credit cards, cash, membership plans, and financing options.

5. Refunds & Returns

• We do not offer refunds for services, packages, or prepaid treatments.
• All gift cards and prepaid packages are non-refundable.
• Retail products purchased in-spa are final sale and not eligible for return.

6. Privacy & Data Security

• We collect personal and medical information solely to provide our services.
• Your data is handled in accordance with HIPAA and other applicable privacy laws.
• We use Aesthetic Record for scheduling and client management.

7. Medical Disclaimer & Liability

• All procedures are performed under medical supervision when required.
• Results may vary from person to person; we do not guarantee specific outcomes.
• You are responsible for following all pre- and post-care instructions.
• Clients are required to sign appropriate informed consent and liability waiver forms before receiving treatment.

8. Intellectual Property

All content on our website, including text, logos, branding, graphics, and images, is the exclusive property of Azalea Medical Spa and may not be copied, distributed, or used without our prior written permission.

9. Client Conduct

We require all clients to maintain respectful conduct toward staff and other clients. Harassment, abuse, or inappropriate behavior will result in immediate termination of services and possible removal from the premises.

• Changes will be posted on this page with an updated effective date.
• We may suspend or terminate services at our discretion if a client violates these terms or acts in a way that jeopardizes the safety or integrity of our business.
